WHAT IS THE DIFFERENCE IN PERSONAL CARE AND HOME CARE SERVICES?

We get this question a lot! There is not a difference in the two, it just depends on how its worded.

WHAT SHOULD I CONSIDER WHEN CHOOSING A HOME CARE PROVIDER?

Choose the agency that makes you feel at home.  Here at A Family That Cares we make sure that all our clients and caregivers are treated as family.  We work closely with our families to build care plans and relationships.

DOES YOUR COMPANY ACCEPT MEDICAID AND PRIVATE PAY?

WHERE ARE YOU LOCATED?

We are located in Northwest Indiana AND Indianapolis, IN

HOW DOES SOMEONE QUALIFY FOR THE PATHWAYS PROGRAM?

PathWays is a managed care program for Indiana Medicaid members that are:

  • 60 years of age and older

  • Are eligible for a full-coverage aged, blind or disabled category (with or without Medicare)

  • Can be receiving long-term support services including:

    • Resides in a nursing or long-term care facility

    • Are approved for an Aged and Disabled waiver

  • Can be on the Behavioral and Primary Health Coordination program

WHAT IF I DO NOT QUALIFY FOR PATHWAYS BUT I HAVE MEDICAID?

Medicaid members that are not eligible for PathWays include:

  • Those under 60

  • Healthy Indiana Plan member of any age

  • Hoosier Healthwise members of any age

  • Division of Disability and Rehabilitative Services waiver members including: 

    • Family Support Waiver

    • CIH (Community Integration and Habilitation Waiver)

  • Traumatic Brain Injury Waiver

  • Room and Board members

  • Medicaid members on the End Stage Renal Disease waiver

  • Breast and Cervical cancer members on MA12

  • Intellectual and developmental disability residents in intermediate care facilities

  • Members eligible for Emergency Services Only due to citizenship status
